Order Form
You may place your order in any of the following three ways:
- Fill in the online order form -located on the Celebratory Materials link- and press the Submit button to be taken to the secure payment page, where you can pay with Visa, MasterCard or American Express.
- If you prefer to pay with a cheque or purchase order, please complete the online order form, click "submit" and print the page that sumarizes your items ordered and total amount due. Fax it to 416-340-4736 or email it to nedic@uhn.ca and include a purchase order #, or mail us a paper copy of your order with your cheque. Don't forget to include your cheque, money order, purchase order # or credit card information with your order.
